Description:
The Customer (being the State of Queensland, acting through the Department of Youth Justice and Victim Support) is seeking quotes for the Staying on Track program.The Customer is looking for a supplier who can:Engage and mentor young people whilst they are in detention and supporting families to prepare for young person-s return.Coordinating and delivering elements of an agreed Service Response Plan (SRP) including goals and practical and wellbeing support and mentoring pre- and post-release from detention for up to 12-months.Participation in stakeholder meetings to develop and review individual SRPs at regular intervals in collaboration with the Youth Justice case manager and other relevant stakeholders.
